Due to patient volume we ask 48 hours notice for refill and sample requests. All prescriptions are now sent electronically to your pharmacy.
When we book appointments for specialists, a referral will be sent to providers' office and one will be sent to the insurance company if they are required. This also applies to phone authorizations. However, if you make the appointment yourself, it will be your responsibility to let us know if a referral is needed. Please allow us 48 hours to do so.
We ask for 7-10 days to fill out disability forms and other related paperwork.
Please bring your insurance card with you so we can verify the information and file an insurance claim for you. Your insurance may not cover the full cost of your services due to deductibles or coinsurance. We will send out monthly statements and ask that you pay your balance within 30 days unless arrangements have been made with the billing department. We do accept Visa and MasterCard.
We no longer take new Workers' Comp cases.
We ask that your copay be paid at the time of service.
